About Home Care Packages

Home Care Packages are a Government-funded program designed to assist those who have complex needs to live independently at your place of choice. Services are designed with you to meet your needs.

To apply for Home Care Package, visit My Aged Care and fill in an online form or call on 1800 200 422

An assessor will visit you to understand your needs, check your eligibility and add you to a waitlist.

When you have been assigned a Home Care Package you can select a provider who will work with you to develop a program of services that meet your needs.

What Caring for You offers:

Personal Care

Maintain your confidence with your daily tasks and routines, look and feel your best. We are here to provide the support that you want in the areas of:

  • Dressing
  • Toileting
  • Other grooming activities
  • Continuation of recommended health and wellbeing routines

We know that having the right level of support in your home helps you live happily and safely. Whether it is domestic services or daily living skills we can aid you with:

  • Payment of bills
  • Meal planning and preparation
  • Respite care when time out is needed

We know that having the right level of support in your home helps you live happily and safely. Here are some additional services we offer to maintain your household.

Nursing and Therapeutic Services

When you have specialised needs, we recognise the importance of staff that have the right skills and attitude to meet your needs.

Our nursing and allied health staff have a wide range of experience including:

  • Allied health services by accredited practitioners
  • Catheter management

  • Diabetes management

Community Access and Social Activities

Everyone should be able to take part in the community the way they want to. We can help you build independence with your daily living skills and provide someone to help you make your chores a fun outing. For you this might be:

  • Participation in community and social activities
  • Shopping, paying your bills
  • Attending medical appointments
  • Going out, perhaps to movies or lunch
  • Gyms, sports, hydrotherapy

Care Management

Home Care Packages require levels of care management. Care management ensures that you receive the care that is right for you and support to ensure that you meet your desired goals.

Caring for You ensures

  • Care is planned with you
  • You have a dedicated person to talk to
  • A co-ordinated budget provides the services you need and is value for money
  • Services encourage wellness and are effective
  • You understand your Home Care Agreement
